Accident Report, Reported by Zheng Hong

Date : May 1, 11:00am

Glider : Flight Design, A7vt (DHV 2),
Helmet : Full face
Harness : Air Bulle
Pilot : Experienced


The wind was strong and not very stable. When he was soaring on the mountain, 1/2 of canopy was collapsed and the other side began to be rolled up, the canopy got into the spiral very fast. The pilot is very experienced, he used the brakes to recover the canopy, but he failed. He was spiraled down for 4 rotations. At the same time, he used one hand to manipulate and hold on the brake to control the collapsed canopy and another hand to deploy out the reserve handle. But very unfortunately and unbelievable, the handle was deployed out from the harness, the reserve didn't pull out from the pocket and to be pressed tightly in the pocket of harness, the reserve could not be opened. He fell down directly on the mountain top. The fall height was around 60m. He was very lucky, when He was very closed to the mountain/around 10m, the canopy was recovered, but he already touched to the mountain. He crushed on several trees and 2 tourists, his waist was injured.

After this accident, we inspected and opened this harness and to find the right reason why the reserve bag was not pulled out. Hereinafter is the operation procedure with the photo pictures for the reference.


Design: The reserve bag is in the bottom of harness. The deployment handle is on the right underneath.


To pull up the handle, and handle comes out from the harness.


But, the reserve bridle can not pull out the reserve bag from the reserve pocket. If you hold and high up the handle higher, the reserve bag is still remained in the pocket and very tight, the harness is also going up.


To get the reserve out from the pocket, you should use 2 hands to open the pocket flap, to pull out the reserve out from the pocket.


The reserve has come out from pocket, the problem is, the reserve bridle is connected to the center of reserve, but this central point is packed in the middle of pocked. You can not pull out the reserve. If the bridle is connected to the right side of reserve, the reserve will be pulled out.




Summarized Reason: The pilot has bought this harness from the dealer for 3 months, the flight time was over 30 hours, actually he flied the glider without any reserve, no any emergency protector, it was very dangerous.


Design-- Designing is not the new model, because the bottom reserve is packed in a tube or tunnel pocket, when you pull out the handle, the reserve should pass this tunnel pocket and come out fully, the deployment bag will be opened. This operation procedure will take a longer time. The rear and front reserve is full open, once the handle is pulled out, the pocket flap will be opened immediately and the deployment bag will be come out immediately.


Usage-- Perhaps, the dealer didn't use the original deployment bag, bridle and handle of this harness attached. This bag/bridle/handle is designed for the harness with Rear or front mounted reserve, not for Bottom reserve. The dealer or pilot can not replace any attachment of the instrument, otherwise you will face the risk, to be the edge of danger.

From this accident, every pilot should understand to pay a great of caution to your flight instruments, to inspect your instrument carefully, don't respect or rely on other people. Remember, flight is your personal safety, you should be responsible for your own life.
